INCIDENT REPORT
Use the form below to submit an incident and/or ejection report.
Submit a separate report for each person given a warning (Yellow Card) or ejected.
Reports should be submitted as soon as possible after the incident (no later than 24 hours).
Tracking participant incidents is important for several reasons:

Game Safety: Ejections can be a result of dangerous or unsportsmanlike behaviour, tracking these individuals ensures the safety of other players and coaches on the field.

Discipline: Yellow Cards and Ejections are a form of discipline for unacceptable behaviour, tracking them helps to maintain the integrity of the game and ensure fair play.

Record Keeping: Keeping track of Yellow cards and ejections allows for proper record keeping, which can be used for disciplinary hearings or appeals.

Prevent Repeat Offences: By tracking these individuals, Softball BC can monitor repeat offenders and take appropriate action to prevent future incidents.

Fairness: Knowing who has been warned or ejected can help ensure fairness by preventing these individuals from continuing to participate in the game, giving an advantage to the opposing team.
Once a report is submitted, the Provincial Umpire-in-Chief will review the form and forward it to your DUIC.
